Information for Groups Required to Provide Insurance Coverage
In order to issue a Certificate of Insurance (COI) the insurance company is asking that the COI form be completed and returned to them. Once they have this form completed a Certificate of Insurance will be issued.
To keep things simple all requests for a COI will go through one person. At the present time it will be the NYN Group Records Coordinator.
Each group or AIS or District requiring a COI should complete a Certificate of Insurance Request form and submit that form to the current Group Records Coordinator. This can be emailed to firstname.lastname@example.org or mailed, address is located in Northern HiLights.
Once received it will forwarded to our insurance agent who will issue the certificate. This will be returned to the group.
